First the issue is not with the extension detection. It detects the proper file ext once one applies a little massage.
Let me set up the background:
I added over 100 folders each containing one mht files and normally one but could contain a max of four flv files some of which had missing extensions. Some of the folders had . (period) in the folder name eg Bob Doe M. D. or vs. These folders "seem" to cause a problem, but it is not exclusive to these folders but unusual non alphanum may be causing same issues .
So three outcomes may result:
1 - the folder contents with dot names is ignored
2 - All folders are included but double .flv.flv are preview assigned to all flv files with that ext. Missing flv files are assigned one flv correctly
3 - Select by extension with blank does not select files with missing flv ext. One must remove all mht files and all identified flv ext files leaving only unidentified ext files before preview and rename will work properly.

Yes, I am aware of the problem and it will be fixed soon.

Untick the option "Append to the original filename" in the "Extension" rule to prevent adding the second extension to files, e.g. .flv.flv.
3 - Select by extension with blank does not select files with missing flv ext. One must remove all mht files and all identified flv ext files leaving only unidentified ext files before preview and rename will work properly.
Sorry, I don't understand this one. "Select by Extension" option selects all files which do not have an extension, it works as expected.

I retried the test unfortunately not with the original files since these files have been cataloged and spread out over a myriad folders.
Seems to work fine when "Untick the option "Append to the original filename""
But if I recall correctly 3 above only occurred when I added the folders from an additional directory level above eg
Video/documentaries instead of documentaries. Can't seem to duplicate error at present, If it turns up again I'll let you know.
